This document grants permission for the coach to use the client's feedback, image, or personal story publicly. It ensures that the client agrees to allow their testimonial or case study to be published by the coach.

1. Consent for Use of Testimonial or Image

I, the undersigned, give my consent to [Coach Name] to use my testimonial, feedback, image, or personal story for promotional purposes. I understand that this may include use in:

  • Websites

  • Social media platforms (e.g., Instagram, Facebook, LinkedIn)

  • Marketing materials (e.g., brochures, newsletters)

  • Other public platforms or promotional content

2. Permission to Publish the Testimonial

I grant permission to [Coach Name] to publish my testimonial, image, or personal story in the following formats, including but not limited to:

  • Online platforms (e.g., Coach's website, social media pages)

  • Printed materials

  • Advertisements or promotional videos
