This Release of Liability, Waiver of Claims, Assumption of Risk, and Indemnity Agreement ("Agreement") is made and entered into as of [Date] by and between [Zipline Company Name] ("Company"), a [State of Incorporation] corporation, with a principal place of business at [Company Address], and [Participant Name] ("Participant"), residing at [Participant Address].
WHEREAS, Company owns and operates a zipline tour located at [Zipline Location] (the "Premises"); and

NOW, THEREFORE, in consideration of the foregoing premises and the mutual covenants contained herein, the parties agree as follows:

4. Medical Conditions. Participant represents and warrants that Participant is in good health and has no physical or mental limitations that would preclude safe participation in the Activity. Participant further represents that Participant has disclosed all relevant medical conditions to a healthcare professional and has been cleared for participation in strenuous physical activities.
5. Indemnity. Participant agrees to indemnify, defend, and hold harmless Company, its officers, directors, employees, agents, contractors, and volunteers (collectively, the "Indemnified Parties") from and against any and all claims, demands, losses, damages, liabilities, costs, and expenses (including reasonable attorneys' fees) arising out of or in any way connected with Participant's participation in the Activity, including but not limited to:
6. Binding Effect. This Agreement shall be binding upon and inure to the benefit of the parties hereto and their respective heirs, successors, and assigns.
7. Entire Agreement. This Agreement constitutes the entire agreement between the parties with respect to the subject matter hereof and supersedes all prior or contemporaneous communications, representations, or agreements, whether oral or written.
8. Severability. If any provision of this Agreement is held to be invalid or unenforceable, such provision shall be struck and the remaining provisions shall remain in full force and effect.
